Throughout a child's life, a child will undergo many challenging transitions as they move to new age groupings, new environments and new schools. Supporting these transitions, through effective communication and collaboration, we can significantly improve outcomes of the transition process. The following document is a summary of information prepared by each child's current educator/s and family and reflects current understanding about the child, including possible ways to settle and support into their new surroundings.
